Do you want custom made or pre-built that will fit your needs. If you're having custom made by the an Amish maker I would recommend going to see then with pictures and detailed sketches or drawing with dimensions. I just built a house with all Amish made cabinets and doors from someone in OH. Keep in mind they don't drive, or have email, limited phone use, sometime they get faxes. I used snail mail for most things, after I visited his shop and met with his finisher down the road. He hired a drives to bring him to MI with my cabinets and spent 2 days installing. They have amazing quality, priced compares to high end cabinets but they are much nicer.
I did meet a furniture maker while picking up a motorcycle a couple years ago in Pontiac. I saw a gorgeous table set but it was $20,000.
Custom furniture makers are expensive....
There are some very good furniture makers sold in stores, check out 5 Hands. They make some amazing tables using reclaimed wood.
Anything by Hooker is good, Baker or Henderdon are all good quality products but fairly expensive.
