Can a MAF fail 2x

long story short like 3 years ago I bought a MAF cuz I was getting a lean code. now I'm getting the same code again. Car is an 01 Escort 2.0

Try cleaning it with MAF cleaner.. available at your local parts store.

Could be a vaccum leak. Likely a dirty maf. what type of air filter do you use ?

opened the hood and looked with my eyes. is there another way?

Spray carb cleaner around areas of vacuum i.e hoses, manifold, gaskets, pcv. If you have a scanner, watch your Short term fuel trim values, and see if they go negative when u spray

can also fill the system with smoke if you have access to a smoke machine
