Bad operator right there. You never telescope your boom once a pick has begun, at around 1:50 in the vid you can see him extending his boom for more reach. That should have been tested on a dry run first, all operators know you have to do that first, to make sure you can safely hoist the load.

The KARE11 story reports that the crane company said computer error is to blame for the mishap. They said there was a lot of margin between the pools weight, and what the crane could handle. The crane company is taking care of the damages.
