Issue with rockers and Hydrulic lifters

The exhaust valve on cylinder number 4 has given me some issues. Here is the issue and how I am setting the lash.

Ok so I adjust the lash and fire the car, runs good and idles smooth. Now after it warms up you hear a "Pop" and the rocker on the exhaust valve comes loose, car runs like shit and I shut it off. I adjust the lash this way, I open the intake valve fully then I tighten the exhaust valve until the push rod does not lift up or down then I spin the poly lock 1/4 turn. Then set the allen screw then adjust it one more 1/4 turn.

Lifter looks fine, dog bones look good also. Anyone else have this issue? It seems the more I adjust it the longer it hold before it pops.
 
The lock retainer broke and it caused the pushrod to jump onto the lip of the lifter.

Adjust Intake when exhaust just start to open, Adjust Exhaust when Intake opens fully the njust starts to close.

Also preload that lifter more. I usually do 3/4 to 1 full turn

I have never had an issue and this problem was because of the retainer clip broke. Its all good now.
